Declaration Form

The Declaration Form will need the following information:

• The age and date of birth of your child.
• The language your child speaks.
• Any allergies or additional needs your child may have.
• Contact details for the person dropping off and collecting your child from the airport or the airline they may be transferring to.
• A contact telephone number for parents or guardians at the departure and arrival airport. This may be needed in case the flights are affected by disruption.
